The Request for Leave of Absence Form (PDF) is also available in the HR Forms Library. Faculty and staff can view their leave balances in FlashLine, by going to the Employee Dashboard.
¾ÅÓÎÓéÀÖ¹ÙÍø also offers a Leave Donation Program, through which employees may transfer sick/vacation leave to other employees who experience a continuing disability due to non-work related illness or injury.
